If a player is injured on the field, beyond the usual bumps and bruises, then it is important to lodge an injury report. If the injury is a head injury then there is extra paper work to fill out. If the player/parent wishes to lodge an insurance claim then again, there's more paper work to complete.
If a player is injured the team manager should complete the following form and provide a copy to the parent, the Secretary of the club and keep one for team records. If a player receives a head injury please complete the following form and provide a copy to the Secretary of the club and keep one for your team records. You will also have to complete a Head Injury Pro Form (below) for a doctor to complete before the player can return to playing. This form also has information about head injuries on it. If a player receives a head injury please complete this form and give it to the player/parents. The player/parent must have their doctor complete the form and return it to you before the player can return to playing or training. Once the form is returned please pass on two copies to the Club Secretary. If a parent/player would like to make an insurance claim they need to fill out the following form. There is a section for the Club Secretary to complete so please forward the form to the Secretary once the parent/player has completed their section and then the form will be forward onto Penrith JRL for processing. Once this process is complete it is in the hands of Penrith JRL and the parent/player should make contact with them on 4725 6401 or [email protected] for updates on the claim. If an incident occurs either on or off the field at a junior league ground and you think it warrants reporting to the Penrith JRL then please complete the following report and submit to the Club Secretary by 9am Monday following the incident.
